We offer several important forms for the 24-hour services we provide in Southwestern PA. Bujanowski Towing understands that ‘the paperwork’ can sometimes be the hardest part of just about any project. Credit Card
Authorization Form
Our Credit Card Authorization Form supplies us with the basic contact details for the person whose credit card will be charged for our services. The cardholder authorizes Bujanowski Towing to charge for payment of services and referenced invoices. Customers using debit cards may also supply bank card information using this form.
Customer
Contact Information Form
The Customer Contact Information Form gives us a snapshot of your details on how best to contact you and where, along with billing details. You can use the form as a new customer or update your records as an existing customer. Click the button below to download the one-page form. Return the completed form to Bujanowski Towing using email or mail it to us so we have the most accurate information for your account.
Inventory Authorizations:
Charleroi or Monessen
These forms cover the release of items from vehicles received by our impound lot locations in either 341 Liberty Avenue in Charleroi, or 1800 Schoonmaker Avenue in Monessen, PA. It is important that you use the correct form. Click to download the one-page form:
Private Property Authorization –
Vehicle Removal
With this form, you give Bujanowski Towing permission to remove vehicles from your property. Please fill in the basic vehicle information, such as the year, make, model, color, VIN, plate, and your address. We will need the property owner’s name, address, phone, email, and license number. With this record completed and signed, we then have your legal approval to remove the identified vehicle. Click to download the one-page form:
Release Authorizations:
Charleroi or Monessen
Much like the inventory authorization form, these cover the release of vehicles from Bujanowski Towing. Customers can designate or authorize another person (with proper ID/valid driver’s license) to remove the vehicle from our care/location. Please use the form for the correct location.
